I have a patient who is fasting Ramadan (which means hes NPO from 5:30 AM till 7:30 PM) and has a dental infection. I originally prescribed Amoxil 500 mg TID, but then changed it to 1 g BID to suit his fasting. Would that dosing regimen still work? Or would I have to switch to something like Pen VK 600 mg BID or Clavulin 875 mg BID? I tried looking up BID dosing of Amoxil in the literature but I could only find it in combo with other meds for treating H. Pylori.Thank you for any suggestions

The MAC of amoxicillin will drop below a therapeutic level if you use BID dosing. With h.pylori, it is alternated with other meds.If you want a Q12 dosing, consider Duricef which has a similar spectrum. Rx: Duricef 500 mg; One Q12H.Or, Biaxin is an erythromycin with Q24 dosing.Rx: Biaxin XL 500 mg; Take two Q24HZithromax is another.Rx: Z-Pack, which is Zithromax 250 mg in a blister pack that gives 500 mg for the first dose and then 250 Q24H.Levaquin is supposed to be a Q24 dose, but I have not been thrilled with the results for dental infections and I use it Q18 when it is the drug of choice. Rx: Levaquin 750 mg; One Q24HorRx: Levaquin 500 mg; One Q18H

I think 1 gm BID is a fairly standard dosing.....don't worry. However, the Quran DOES make allowances for being sick during Ramadan.
